1) Natural Calamities viz. Flood, Cyclone, etc.

We are the parents of the undersigned students. We beg to state that, and as you are well aware, our locality has been greatly affected by the recent cyclone. The school was also closed for several days for this purpose, which affected the scheduled completion of some of the chapters of the papers in class.

We understand the quarterly examination is scheduled to begin from (date). You would appreciate the students' studies have been distracted by the cyclone and during the days the school was closed.

Affidavit Format for New Electricity Connection

While applying for new electricity connection, the applicant has to submit to the Electricity Company / Board, an Affidavit along with application form and other supporting documents viz. applicant's proof of identity & address, proof of occupancy (purchase deed / rent agreement with noc from landlord), passport photograph, etc.

An affidavit has to be signed by the applicant in the presence of two witnesses who shall sign too, and has to be attested by a Notary Public before submitting it to the Electricity Company.

Either prepare the affidavit in a format prescribed by the Electricity Company or take the help of an advocate for this, otherwise you may refer the below template.

5. that the electricity connection sanctioned in the said premises in my name for using electricity for my building for residential / commercial purposes shall be used for the same purpose as sanctioned;

7. that in case of electricity is to be used for a licensable trade, I shall obtain N.O.C. / License from the Competent Authority as may be required;

8. that I undertake to abide by the Electricity Act, 2003 and relevant electricity laws of the State of .................. as may be applicable for this connection from time to time;

9. that (Name of the Electricity Company) shall be at liberty to disconnect, without prior notice, the electricity supply including disconnection of electricity of my other premises, and may may take all possible action as per law for recovery of dues including penalties or additional charges, under the following circumstances:
(i) failure on my part to pay the electricity bills on time by the due date or extended due date;
(ii) failure on my part to abide Electricity laws of the country applicable as applicable for my electricity connection;
(iii) the electricity connection is found to be used for any purpose other than for which it has been sanctioned; and
(iv) violation of the terms and conditions mentioned in the application form for electricity connection.
